I think it’s pretty rare and the most famous example was a game where Washington (I think) took a timeout before trying a game winning kick, and then took another one so they got the penalty and the kicker missed by being like 2-3 years short lol. And it may have been a 15 yard penalty back in the day but whatever it was, it cost them the game and afterwards their coach (Joe Gibbs Amat the time I think) said he had no idea that was a rule lol
