Cheating a little bit because it's a nine track EP of sorts, but "Mono" by Fightstar would get the nod from me. Absolutely stunning six minutes.

It's probably been said, but Dendron is a phenomenal closing track. Ode We Will Bury Ourselves of off Pneuma by Moving Mountains kills it too.
The first two that came to mind were "...but home is nowhere" by AFI and "*fin" by Anberlin. Gorgeous tracks.
Dendron - The Hotelier is an obvious one for me also really like the end to Antarctigo Vespucci's latest, I See Faliure Dark Places by Gaslight too
I think those two are their best closers too but I really like Vital's closer. If I didn't love "Self Starter" so much it'd be my favourite song on there.

There are probably millions more though. I find that my favourite song on an album is the closer about 50% of the time.
