I've had it happen a few times, but only at places where I was spending way too much money.

The rule is essentially that if the you should stir unless the drink involves a strong mixer (dairy, egg white, simple syrup, fruit juice, etc). Martinis, manhattans, negronis, etc. should be stirred. Very few places will do it unless they really care about their cocktails.

I think it's just easier/faster to chill a drink, get it diluted faster, etc.
