I don’t know the extent of ligament tensile strength of a naturally healed ACL vs a reconstructed ACL via graft (which is almost every ACL repair, but there are several graft options) because we have limited data. I’ve heard physicians say the grafts are stronger than natural ligaments but I’ve never seen studies with full numbers to support this. Some studies show 10-30% re-tear rates for repaired ligaments

However, non-surgical rehab mitigates a significant amount of strength loss and muscle atrophy. We obviously have limited (but not insignificant) research on non-operative repairs of ACL tears, especially especially in professional athletes, but quad/hamstring/hip strength is what protects the knee ligaments because ligament is only passive stability to the knee joint so it’s reasonable to assume proper strengthening would be adequate.

My main point is that I wish there was a transition to more conservative treatment. If someone tears in-season they are not coming back at all until the next year. But if someone had an injury earlier in a year, why can they not try bracing for 6-weeks and then rehab if their stability improves? It is a surgery that can be delayed so why not try 2-3 months of rehab if it’s not going to change their return timeline to the sport?

 

I’m reading on ducks Reddit that before the combine Franklin had the flu and lost weight, which resulted in the brutal combine. Maybe that had something to do with it, maybe not.
