The Appalachian Mountains go through Western PA, but I don't know anyone around here that would describe as being from "Appalachia". I think more people would agree on the "rust belt" if you HAVE to give it a regional identity.

have a friend whose family is from west virginia and this is how he uses it

Randall I literally grew up in Western PA and know a lot of people from Ohio, West Virginia etc, the accents are not the same.

 

Rust Belt literally just means “Non coastal US cities that declined because of the loss of mining industries”. It’s not a region and it doesn’t refer to one culture. St Louis and Pittsburgh are not the same geographical or demographic region so that term is not a very good regional categorization
